Monday	12:00 PM - 5:00 PM
Tuesday	12:00 PM - 7:00 PM
Thursday	12:00 PM - 7:00 PM

Fees
$175 per 50-minute session. I have deliberately chosen not to join any insurance panels. However, my work is reimbursed as an out-of-network benefit by most insurance plans. Related to this decision, when appropriate fee adjustments can be made by mutual agreement.

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash or check accepted for payment.

Cancellation Policy
If you do not keep your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
